Using Jotform Templates

  1. Click the blue Create an Order Form button.
  2. Scroll through the templates and click the Use Template button of an order form that matches your needs. In this example, we pick the Product Order Form.

  3. Select the ADD YOUR LOGO option at the top of the form, then upload your logo using the Upload File button on the right.

  4. Click the Payment Settings link to configure payment details and products. This option would not be displayed if you already have payment configured.

  5. Select the Create Product button to add your products.

  6. Click the Basic tab and fill out the form with details of your product, including the Name, Price, and Description. Then, using the CHOOSE IMAGES button, select the desired product images.

  8. Navigate to the Options tab, then click Add for inserting a Add a Quantity Selector.

  9. Click the Please Select dropdown, select either Dropdown or Textbox, fill out the form, and click Save Quantity.

  10. Add as many products as you wish. You may also click on a product on the form and delete it by clicking its Delete icon.
  11. Double-click on any form section and use the extra options to the left to edit its values.

  12. Once you are pleased with the form, click Settings at the top and name your form.

Building Forms From Scratch With Jotform

  1. Click the blue Create an Order Form button.
  2. Click Create Form on the top left corner, then on the next window, select Start from Scratch.

  3. Select a layout. For this example, we choose Classic Form.
  4. Upload your logo by dragging it to the Upload Organization Logo field, input your Organization Name, and click Save.

  5. Click the Add Form Element to your left to expose all elements that may be used in your form.

  6. Drag the elements you need from the left pane to your form.
  7. Double-click on any form section and use the extra options to the left to edit its values.

  8. Select the Payments tab in the Form Elements menu, then drag your preferred payment option to the form.
  9. This should open up a pane on the right where you may click Connect and follow the wizard to configure the payment.

  11. Lastly, switch to the Publish menu, then Copy Link for sharing.

How to Create an Online Order Form on WordPress

If your business website is on WordPress, you can also create an online order form from your website. You can install an e-commerce plugin and get an order form. However, if you want an order form without having a traditional e-commerce website, you can download form plugins on WordPress.

One such form plugin is WordPress’s form plugin, WPForms. Installing and creating an order form is not easy.

Install and Activate WPForm

  1. Go to your WordPress dashboard
  2. Find Plugins from the left sidebar and click on it. You’ll be directed to your plugins page; click Add New at the top.

  3. Search for WPForms at the search bar and click on it when it comes up.
  4. Click Install to install the plugin and Activate it when you’re done installing.

  5. WPForms will be added to your list of plugins and on the sidebar. Click on it.
  6. Select Settings from the dropdown menu.
  7. Click Connect with Stripe (this may be other payment options available to you), and follow the wizard to complete the process.

Create an Order Form Using WPForms

  1. Click on WPForms on the sidebar to open it, and click Add New.

  2. Name your form and select Billing / Order Form or any other template. You’ll get different fields to edit the form on the left and a preview on the right with prefilled standard information.
  3. Add your items and their prices in the Available Items field. You can edit the field as you wish. For instance, you can add product images if you want to.

  4. You can add other fields by dragging them from the left side and dropping them in the preview section. And you can edit the fields as well.

  5. Click the Payment tab, then select any available payment. In this case, we select Stripe.

Publish the Form on Your Website Page

  1. In your WordPress dashboard, hover WPForms and click All Forms.
